There are 6 ways of requesting a repeat prescription.

By using Patient Access* (you must have registered and provided identification to use this)

By returning the white repeat section of your last prescription.

By completing a prescription slip at the reception desk.

By faxing a written request to (0117) 9626663. 

By asking your local pharmacy to make the request on your behalf (please check with your local pharmacy to see if they are able to do this)

By writing to us and enclosing a SAE.

 Please note we DO NOT take requests over the telephone.



*What is Patient Access? 

Patient Access allows you to access your practice online.

Please call in to the surgery with proof of address to register for this service

Patient Access includes a number of optional features, including:

  • Checking, booking and cancelling appointments.
  • Checking your medication and ordering repeat medication.
  • Updating your contact information, including mobile phone number and email address.
